How to Apply For a Health Card (Tarjeta Sanitaria) in Andalucia?

A substitute card can be applied for via the website provided below with or without a digital signature. You can also apply in person at your health centre. The staff should assist you.

Not available online for the unemployed, some types of work contracts and those on a non contributory pension.

You will need to print the form and present the documents in person at your local health centre if it is the first application:

 

Application

 

ID (resident certificates and passport)
ID for children of 14 years or over (libro de familia). Children under 14 documentation that accredits that they are beneficiaries of the person insured with the social security.
Proof of being in the social security system (document and or social security number).
Certificate of empadronamiento (padrón).

Once registered you can ask for a temporary certificate until you are in receipt of the health card.
